As carers of loved ones, it is important and reassuring to plan ahead and have necessary documentation completed for if and when you may need it. This is also important for you as a carer, in case a family member has to step in to assist.
Join us for a session with ACT Health’s Advance Care Planning team, where we will learn
- Understand the importance and benefits of Advance Care Planning
- Be comfortable with the conversations we all need to have and talk early and talk often
- Think about what “living well” means to you and perhaps start to develop your own Advance Care Plan
- Local documents and processes, how to lodge documents, where to find out more etc.
This session will be conducted wholly online however there is an option to phone into the session if you aren’t able to access internet.
This activity is open only to carers registered with Carers ACT. Booking to attend is essential through the link above so that you receive the login information.
By attending and participating in this session, you consent to video, images and audio of yourself and those you are responsible for being used in this manner. As this session is being live streamed, participants are reminded to arrive before or on time. The session will commence once the facilitator is available and as close to the advertised time.
