As carers of loved ones, it is important and reassuring to pain ahead and have necessary documentation completed for if and when you may need it. This is also important for you as a carer, in case a family member has to step in to assist.
Join us for a session with ACT Health’s Advance Care Planning team, where we will learn
– what is an advance care plan, the different types and what they legally enable a carer to do on someones behalf;
– what to consider as part of the plan for you or the person you care for;
– how to have a conversation with loved ones about your wishes (tips and tricks);
– how plans are activated;
– and more.
You are welcome to bring other family members to this session. This session will also be live streamed on the internet.
Carers ACT will be live streaming this session. If you are unable to attend on the day but would like to view the live stream of the session from your electronic device (such as mobile phone, tablet, or computer), please book a “Streaming” ticket. If you select a “Streaming” ticket, you will receive a separate email from the Group Programs Team prior to the advertised date and time with a link to click on the session.
By attending and participating in this session, you consent to video, images and audio of yourself and those you are responsible for being used in this manner. The session will commence once the facilitator is available and as close to the advertised time as practical.
