NDIS Support Coordination

Many people find it challenging to implement the funded support and services included in their NDIS plan.

At Carers ACT, we work to assist you get the most out of your plan and choose the services best able to meet your needs.


What is Support Coordination

Support Coordination is an NDIS funded support that is designed to help participants make the most of their NDIS funds. Once an NDIS plan has been approved, participants work with Support Coordinators to determine how their funds will be spent, choosing the best options for services and connecting with service providers.

You get to choose your own Support Coordinator.

Who Gets Support Coordination?

Not every participant is funded for Support Coordination but everyone can ask for it. If you want Support Coordination in your Plan, be sure to mention it in your planning meeting.

What Carers ACT Can Do For You

We have a deep understanding of how disability can impact your everyday life and goals. We know the disability and community sectors back to front so you don’t have to explain things and repeat yourself to us.

We have spent the past three years participating in the NDIS so we understand how the system works and how to get the best out of your Support Coordination.

Our Support Coordinators will work alongside you to support and empower you to make your NDIS plan active. We will assist you to:

We take direction from you about how you want to manage your plan. You might choose to allocate some responsibilities to us, while you take others. The choice and control is yours. Together, we can work toward your goals.
